For Members

Lab Card Select is part of your health benefit plan. The Lab Card Select Program offers you and your eligible dependents discounted outpatient laboratory testing when your testing is sent under the Lab Card Select Program to a participating Quest Diagnostics laboratory. To use this voluntary program, the testing must also be ordered by your physician, covered and approved by your health benefit plan.

What is Lab Card Select?

Lab Card Select is an outpatient laboratory discount program for insurance companies and payers that market high-deductible medical plans, medical savings accounts and defined contribution plans. By adding the program, payers provide members with the ability to save money by directing their laboratory testing to our high-quality laboratory.

The Program is simple

1. Lab Card Select members show their doctor’s office their Lab Card Select card or Healthcare Card with the Lab Card Select logo and request that their specimens be sent to a participating Quest Diagnostics laboratory for testing.
2. If the doctor customarily collects specimens in the office, he/she simply sends those specimens to a participating Quest Diagnostics laboratory for testing. If not, the member can go to a Lab Card Select contracted collection site, if available.
3. Quest Diagnostics sends a courier to pick up the specimen, completes the testing and sends results to the physician, usually within 24 hours.
4. Quest Diagnostics then determines the eligibility of the member and edits, formats and transmits the claim to the payer. The payer adjudicates the claim and communicates the information to Quest Diagnostics. Quest Diagnostics bills any unpaid balance the members.
