Information for Physicians

Lab Card Select is a discount program that offers low-cost outpatient laboratory testing when specimens are directed to Quest Diagnostics.

How the Program Works
Your patient should present his/her Lab Card Select Card or healthcare card with the Lab Card Select logo. In order for your patient to benefit from the cost savings of Lab Card Select, their specimens must be sent to Quest Diagnostics for testing. For courier pickup call Quest Diagnostics at 1-800-750-1253. In most cases, test results will be faxed the next morning – before your office opens. If you are unable to collect your patient’s specimens, you can send them, along with a test requisition, to an approved Quest Diagnostics patient service center. At the patient service center, specimens will be collected by a trained medical professional and sent to Quest Diagnostics for testing.

If you do not have Quest Diagnostics test requisitions, you can use any prescription pad that has your fax number on it. To order personalized Quest Diagnostics test requisitions, call us at 1-800-750-1253.

If your patient’s specimens are sent to a laboratory other than Quest Diagnostics, they will not receive the savings this benefit provides. The Lab Card Select Program includes most outpatient laboratory tests. STAT tests, bleeding time tests, and tests performed while your patient is in the hospital are not covered by Lab Card Select.

Client Services
Quest Diagnostics’s client services staff is available to assist you weekdays from 7 a.m. to 7 p.m., and Saturdays from 7:30 a.m. to 4 p.m. CST. Emergency calls will be answered 24 hours a day, 7 days a week. By calling client services at 1-800-750-1253, you can order additional tests or obtain information on abnormal ranges, results reporting, supplies or billing. Furthermore, we have established a network of in-house medical and technical experts who are available to provide you with specific test information.

Quest Diagnostics is a fully accredited and certified laboratory, licensed by the U.S. Department of Health and Human Services under the provisions of the Clinical Laboratory Improvement Amendments of 1988. The laboratory is also accredited by the College of American Pathologists and certified by the Substance Abuse and Mental Health Services Administration (SAMHSA), formerly NIDA.
